主人公イーサン・ウィンターズは、3年前に突然行方不明となった妻ミアをずっと探し続けていた。2017年7月のある日、ミアから「迎えに来てほしい」というメッセージが送られてくる。イーサンはメッセージの情報を頼りに、ルイジアナ州ベイカー農場の跡地に赴く。

イーサンは廃墟となったベイカー邸でミアと再会するが、ミアは次第に様子がおかしくなっていき、ついにはイーサンを殺そうと襲いかかってくる。やむなくミアを打ち倒すことで難を逃れたイーサンだったが、その直後に背後から謎の人物に襲撃され、意識を失う。やがて目を覚ましたイーサンが見たものは、おぞましい料理の乗った食卓を囲むベイカー家の住人の姿だった。

ベイカー家の主人で不死身の怪物ジャック、その妻で羽虫を操るマーガレット、トラップに凝っている猟奇趣味の長男ルーカス、そして車椅子に座りほとんど動かないにもかかわらず屋敷の異なる場所に出没する謎の老婆……。彼らにより屋敷に囚われてしまったイーサンは、電話でコンタクトを取ってきたベイカー家の末娘ゾイの協力を得ながら、狂気に満ちたベイカー家の館からの脱出を試みる。

長い探索の果てに、ミアやベイカー家を襲ったバイオハザードの真実が解き明かされ、謎の少女・エヴリンの特異菌と精神感応により、ミアやベイカー家が操られていることが判明する。そしてイーサンは、すべての元凶である屋敷に潜む邪悪な存在（レジデント イービル）である、エヴリンと対峙することになる。

For three long years, Ethan Winters never stopped searching for his missing wife, Mia. Then, in July 2017, a cryptic message arrives from her: "Come get me." This single plea leads Ethan to a derelict plantation in Dulvey, Louisiana.

Inside the rotting Baker mansion, Ethan finds Mia—but the reunion quickly turns into a nightmare as she attacks him with unnatural ferocity. Though Ethan manages to subdue her to survive, he is instantly ambushed from the shadows and knocked cold. He awakens to a macabre sight: bound to a chair, facing the twisted Baker family at a dinner table served with grotesque human remains.

The family is a gallery of horrors: Jack, the unkillable patriarch; Marguerite, a matriarch commanding swarms of bugs; Lucas, a psychotic son obsessed with lethal traps; and a mute, wheelchair-bound matriarch who seemingly teleports throughout the estate. Held captive in this house of madness, Ethan forms an uneasy alliance with Zoe, the family's estranged daughter, who guides him via phone lines as he fights to escape.

As Ethan uncovers the grim reality of the outbreak, he learns that Mia and the Bakers are puppets, infected and telepathically controlled by a genetically engineered girl named Eveline. To end the nightmare, Ethan must stand against Eveline—the ultimate bio-weapon and the true "Resident Evil" lurking within the walls.
